Welcome to our practice. We appreciate the opportunity to serve your health care needs. The following information is intended to provide a brief overview of our practice's policies and procedures, from scheduling your appointment to billing.
Patients are seen at scheduled appointment times. All new patients should arrive 15 minutes prior to your first visit to complete the necessary paperwork before your scheduled appointment time. It is important to arrive on time for your visit. If you are unable to keep your appointment, please call the office as soon as possible to cancel your appointment.
Payment and Billing
We accept most major health insurance carriers. Please bring your insurance card with you so that we can make a copy for your record. It is the patient's responsibility to provide us with current insurance plan information prior to services rendered for accurate billing of services to be filed. We strive to offer our patients excellent medical care and will assist you in receiving maximum benefit from your insurance plan.
We will bill all insurance carriers for services rendered, but we ask that all co-payments and applied deductibles be paid at the time of service. Payment is required at the time service is rendered for all private-pay patients unless otherwise arranged with the office administrator. If there is a balance due after payment is rendered by your insurance company, we will expect your prompt payment of that balance.
We accept Master Card, VISA, Discover, checks and cash as payment for services rendered. Payment for visits due to worker's compensation or resulting from a motor vehicle accident will need to be paid in cash at the time of the visit, and you will need to seek reimbursement from your insurer or legal settlement.
If you require a refill of a prescription, please contact your pharmacy directly, so that your refill request can be submitted electronically. You may also contact our office for prescription refills. If you choose to call us for a prescription refill, please leave the following information on our voice mail system: your full name, date of birth, name of the medicine(s), the prescription number on the bottle, your phone number and the name, address and phone number of the pharmacy. Please allow 48 hours for all refills.
If you are experiencing an emergency, call 911 or go immediately to the nearest hospital emergency room. You may also call our office with questions about unusual symptoms or minor emergencies after hours.
If your emergency occurs after hours and the doctor is not in the office, you may leave a message with our answering service and our system will page him, so that he can return your call.
